Description: DISTRACTION: This newborn was born with Pierre Robin Sequence, which is a condition characterized by a small lower jaw, cleft palate and breathing difficulties. We had tried all sort of conservative measures to improve her breathing including placing her on her stomach, inserting a nasal breathing tube, all of which were unsuccessful. At this point, the parents were faced with the option of a surgical airway (tracheotomy) or distraction osteogenesis, and elected to proceed with the later. This is a procedure where bone cuts are made on either side of the jaw, which are then stabilized with a small plates with an adjustable screw. The bone segments are gradually pulled apart (distracted) by turning an external screw several times a day which enables new bone to form across the gap. The result is to effectively lengthen the jaw and eliminate breathing and feeding difficulties in a short period of time, as little as one week. read more

Description: FACIAL TRAUMA: This patient presented with a significant upper face and nasal injury from a motor vehicle accident. Both orbits and nasal root as well as frontal sinus were fractured. She underwent extensive reconstruction using incision hidden in her hairline, eyelids and mouth. Her facial bones were repositioned with small metal plates, and additional bone grafts from her skull were used to project her nose. read more

Description: NASAL DEFORMITY/RECONSTRUCTION: This patient presented with an old nasal deformity as a result of an assault that resulted in complete collapse of his nasal bones. His initial repair was inadequate. He underwent reconstructive rhinoplasty using a cartilage graft taken from his rib that was carved into the shape of his missing nasal bones. read more

Description: LIP BURN: This young girl presented with a burn to the corner of her mouth as a result of chewing on an electrical cord. She underwent early reconstruction to prevent permanent disfigurement of the corner of the mouth. The reconstruction was accomplished by advancing the healthy portion of the lip over the missing segments. read more

Description: MICROTIA: This patient was born with a rudimentary left external ear (microtia) . He was reconstructed in a series of stages. In the first stage, a cartilage graft was harvested from his rib and shaped into a framework resembling an ear. It was then buried under the skin in the location where his new ear was constructed. In the second stage, the ear framework was elevated from the scalp and the back surface was grafted with skin. Additional stages will include opening up the ear canal and creating a tragus, the prominence in front of the ear canal that offsets it from the face. read more

Description: Reality Bites
This unfortunate boy was bit by a neighbors’ dog, avulsing a segment of the upper lip. Dr. Falk was able to repair the lip by precisely underlining, advancing and approximating the wound edges.
