Dr. Falk at the Center for Facial Plastic Surgery and Laser Skin Care, provides the latest laser treatment for toenail fungus using an Nd:YAG laser by Cutera. This treatment harnesses the power of the Nd:YAG wavelength, delivered in a painless micropulsed mode, to eradicate fungus that lives under the nail bed which is the root cause of this disfiguring nail condition. In just one or two sessions improvement in the appearance of fingernails and toenails is evident.
The signs of toenail fungal infection include thickened and unsightly nails, most commonly the big toe. These nails are brittle with distorted shapes, lack luster, and sometimes are dark or vary in color. Generally, toenail fungus is a cosmetic concern, but there can be itching and secondary infection that compounds the problem. This condition is seen in men and woman, and can be associated with other systemic disease such as diabetes and peripheral vascular disease.
The underlying cause of this condition is colonization of the space under the nail with fungal species called dermatophytes, the most common variety being Trichophyton rubrum. The fungus digests the keratin the protein that composes the nail, which gives rise to the unattractive appearance of the nail. Because of the location being inaccessible under the nail, it is difficult to irradiate the infection once it takes hold.

Treatment for this condition includes medical therapy such as Lamisil™ although it has a 50% success rate at best, and there can be liver toxicity associated with its long-term use. Topical therapy with lacquers applied like nail polish have also been tried but are not very effective either. Lasers are the new alternative and have a very high cure rate. This non-painful treatment is accomplished in a 25 minute session. All toenails are treated regardless of whether they demonstrate outward signs of infection to prevent occurrence of subclinical infection or re-infection. There are no restrictions after treatment. Slowly over a period of 3 months, patients can expect to see improvement in their nails at a rate consistent with that of the new nail growth.
